Privacy Policy
What this website collects, where it goes, and how to have it removed.

This website, jacksonvillehandsurgery.com, is operated by R. David Graham, MD. Dr. Graham practices within Jacksonville Orthopaedic Institute, and the care you receive there is governed by JOI’s own notices, which apply to him as a JOI physician. This policy describes only what this website collects and what happens to it.

What we collect
Only what you type into a form. There are three, and each collects a fixed list of fields that is enforced in the site’s own code — a field that is not on the list is discarded rather than stored.

The appointment form deliberately does not ask for your date of birth, your insurance, or a description of your condition. Those are discussed on the telephone, where the conversation is protected in a way a web form is not.
What we do not collect
- No cookies. This site sets none at all. There is no cookie banner because there is nothing to consent to.
- No advertising, no tracking pixels, no social media trackers.
- No third-party fonts or scripts. Typefaces are served from our own server, so no outside company is told which pages you read.
- We never sell, rent, or trade your information.
Where your information goes
When you submit a form, two things happen, in this order:
- A record is saved to Cloudflare Workers KV, our hosting provider’s storage. This happens first, so that a failure to send email can never lose your request. These records are automatically deleted after 90 days.
- An email is sent to our scheduling team through Resend, an email delivery service. Resend retains message data for 30 days under its own policy, then deletes it.
The email itself then sits in our staff mailboxes, and it is not deleted automatically. We would rather say that plainly than imply a schedule we do not keep: the 90-day and 30-day periods above are enforced by software, but an email in an inbox stays there until someone removes it. If you would like your request deleted, ask us and we will remove it.
Analytics
We use Cloudflare Web Analytics to count page views. It is privacy-first by design: it does not use cookies or any other client-side storage, and it does not track visitors across sites or over time. We can see how many people read a page. We cannot see who you are, and we do not try to.
Security, stated honestly
Everything you send travels over an encrypted connection. Email, however, is not a secure medium once it leaves us — which is why the appointment form’s email field is optional, is labelled as such, and is used only to confirm that we received your request. We will not discuss your care by email.
Your choices
You can ask us what we hold about you, and you can ask us to delete it. We will honor either request promptly and at no charge. You do not have to use any form on this site — you are always welcome to simply call (904) 241-1204.
Children
This site is not directed at children and we do not knowingly collect information from them. A parent or guardian making a request on a child’s behalf should use the phone number above.
Changes to this policy
If this policy changes materially we will update the date at the top of this page. What the forms collect is enforced in the site’s own code and is checked automatically whenever the site is rebuilt, so it cannot quietly drift from what this page says.
